The self-score version of the Faith Identity Test is linked below. Anyone is free to use this test. If you intend to use it for research, please do so under the following conditions.

1. Please do not alter the instrument.
2. Please give credit to the author.
3. Please send a copy of the results of your research to the author of the test at drbillduvall@outlook.com.

I’ve also included an Excel spreadsheet with formulas built in for easy scoring. The same formulas should work in a Google Sheet. Just replace the sample data with your actual data and copy as many rows as needed.
